Misoprostol is a synthetic derivative of prostaglandin E 1 ; a drug that induces contraction of smooth muscle fibers of the myometrium and dilation of the cervix, which helps to remove the contents of the uterus cavity.
Release form and composition
Dosage form - tablets: round, biconvex, white (3 or 4 pcs. In aluminum blisters, 1 blister in paper boxes).
The active ingredient of the drug - powder misoprostol HPMC, incl. misoprostol - 0.2 mg, hypromellose - 19.8 mg.
Auxiliary components: microcrystalline cellulose, castor oil, colloidal silicon dioxide, sodium carboxymethyl starch.
Indications for use
Misoprostol is used in combination with mifepristone for early termination of pregnancy (up to 42 days of amenorrhea).
- Liver and kidney disease;
- Cardiovascular diseases;
- Hormone-dependent tumors;
- Endocrinopathies and diseases of the endocrine system, incl. adrenal dysfunction, diabetes mellitus;
- Diseases associated with prostaglandin dependence, or contraindications to the use of prostaglandins: asthma, glaucoma, arterial hypertension;
- Suspected ectopic pregnancy;
- The use of intrauterine contraceptives (before the appointment of Misoprostol, they must be removed);
- Lactation period;
- Hypersensitivity to the drug.
Dosing and Administration
Misoprostol is intended for oral administration.
The drug is used only in institutions that have properly trained medical professionals.
Abortion scheme: the patient is given 3 tablets (600 mg) containing mifepristone as the active substance, after 36-48 hours - 2 tablets Misoprostol.
Possible pain in the lower abdomen, dizziness, nausea and vomiting, lethargy. In some cases, allergic reactions, fever, itching, flushing of the face are observed.
Misoprostol should be used only in combination with mifepristone, exclusively on doctor's prescription, always under his supervision, in a specialized medical institution that has the ability to provide emergency blood transfusion and gynecological surgical care.
Before taking the drug, the patient should be observed in the hospital, it must be warned about possible side effects. During and after reception, the woman should be under close medical supervision so that in case of massive bleeding or other complications she can be provided with timely medical assistance.
As a rule, after taking Misoprostol, women experience a slight vaginal bleeding, in some cases - quite long. With very early pregnancy, miscarriage is possible after taking mifepristone, but taking Misoprostol is still necessary. After using the drug in approximately 80% of cases, abortion occurs within 6 hours, rarely within 1 week.
After 8-15 days after taking the drug, the woman should undergo a re-examination in the same medical institution. If necessary, an ultrasound examination and analysis to determine the level of human chorionic gonadotropin in the serum. If you suspect a preservation of pregnancy or incomplete abortion, you need to conduct a comprehensive medical examination. If the suspicions are confirmed, they conduct a vacuum with a subsequent histological examination of the aspirate.
During pregnancy, the drug can only be used to interrupt it, otherwise it is strictly contraindicated.
If necessary, the use of the drug during lactation breastfeeding should be temporarily discontinued. You can resume feeding 5–7 days after medical abortion (5 days after taking misoprostol, but not earlier than 7 days after mifepristone).
Smoking more than 10 cigarettes per day, taking barbiturates, antidepressants, anticonvulsants, acetylsalicylic acid, cimetidine, indomethacin, isoniazid or rifampicin stimulates the metabolism of misoprostol, which reduces its concentration in blood serum.
It is recommended that you refrain from taking aspirin and other nonsteroidal anti-inflammatory drugs for 7 days after taking Misoprostol.
