Mercazolil is an antithyroid drug used in hyperthyroidism.
Release form and composition
Mercazolil is produced in the form of tablets: flat-cylindrical, from white with a yellowish or creamy shade to white, round with a facet (in plastic containers or polymer cans of 50 or 100 pcs., 1 container or can in a cardboard box; 10 or 20 pcs., 5 or 10 packs (10 tablets each) or 2 packs (20 tablets each) in a carton box.
The composition of 1 tablet includes:
- Active ingredient: Tiamazol - 5 mg;
- Auxiliary components: calcium stearate monohydrate - 1 mg; talc - 2 mg; potato starch - 98 mg; lactose monohydrate - 94 mg.
Indications for use
- Thyrotoxic crisis;
- Preparing for thyroidectomy.
- Granulocytopenia (including history);
- Pregnancy and lactation;
- Hypersensitivity to the drug.
Dosing and Administration
Tablets are taken orally after meals.
The dose of Mercazolil for children is determined on the basis of 0.3-0.5 mg of the drug per 1 kg of body weight.
Adults with mild and moderate forms of the disease are prescribed 1 tablet 3-4 times a day, in severe form - 2 tablets with the same frequency of administration, after the onset of remission - 0.5-2 tablets per day.
During the period of Mercazolil therapy, side effects may occur on the part of some body systems:
- Peripheral and central nervous system: in rare cases - headache, polyneuritis, neuritis;
- Endocrine system: hyperplasia of the thyroid gland;
- Digestive system: rarely - taste disorders; in rare cases - nausea, vomiting, abnormal liver function;
- Hematopoietic system: rarely, hematopoietic disorders (agranulocytosis, leukopenia); in isolated cases, generalized lymphadenopathy;
- Allergic reactions: skin rash; rarely fever; in isolated cases - lupus-like syndrome.
During the period of Mercazolil use, it is recommended to monitor the lumen of the trachea, the level of thyroid-stimulating hormone, and the picture of peripheral blood.
With too early withdrawal of the drug, a relapse is possible. Treatment is not recommended without regular medical supervision.
Patients with a significant enlargement of the thyroid gland, narrowing the lumen of the trachea, tiamazole should be used briefly in combination with levothyroxine, because with prolonged treatment may increase goiter and even greater compression of the trachea.
If a throat, dysphagia, furunculosis, signs of stomatitis appear suddenly during treatment, and when the body temperature rises, you should immediately stop taking the medicine and consult a doctor.
7-14 days prior to the operation, Mercazolil should be canceled and continue preparation for strumectomy with iodine-containing preparations.
With iodine deficiency, the effect of thiamazole increases, and with an excess it decreases.
Cancellation of Mercazolil is required in cases of severe epigastric pain, jaundice, severe weakness, persistent nausea or vomiting, generalized skin rash and itching, subcutaneous hemorrhages or bleeding of unclear origin.
After the end of therapy, late hypothyroidism occurs in rare cases, which is not a side effect of the drug, but is associated with destructive and inflammatory processes in the thyroid tissue that occur within the framework of the underlying disease.
The effect of tiamazol is enhanced when used in combination with beta-blockers (especially during the preparation for subtotal thyroidectomy), lithium preparations, amiodarone, reserpine.
The antithyroid effect of thiamazol is enhanced while it is used with gentamicin.
When using tiamazol in combination with metamizole sodium, sulfonamides, myelotoxic drugs increases the risk of leukopenia; with folic acid, leucogen, the risk of leukopenia is reduced.
