Dosing and Administration
The recommended daily dose of Androgel is 5 g (which corresponds to 50 mg of testosterone). The drug is used 1 time per day, at the same time, preferably in the morning. Depending on the clinical and laboratory parameters of the daily dose can be adjusted, but it should not exceed 10 g of gel.
In order to adjust the dose of the drug, it is necessary to determine the concentration of testosterone in the morning, before applying Androgel, from the third day after the start of treatment.
The gel should be applied to dry, clean and intact skin of the forearm, shoulders and / or abdomen. It is impossible to apply the gel on the genital area, since it contains a large amount of ethanol and may cause local irritation.
The most frequent side effect from the use of Androgel is skin reactions at the site of application, dry skin, erythema, acne. During clinical trials, the following adverse effects were noted:
- On the part of the urogenital system - mastodinia, changes in the prostate gland, gynecomastia;
- On the part of the blood and lymphatic system - changes in lipid levels, polycythemia;
- On the part of the cardiovascular system - increased blood pressure;
- From the nervous system - hyperesthesia, amnesia, dizziness, headache, paresthesia, mood changes;
- On the part of the digestive system - diarrhea;
- Dermatological reactions - urticaria, alopecia.
The drug Androgel should be used only if testosterone deficiency is accompanied by such clinical manifestations as regression or underdevelopment of secondary sexual characteristics, impaired lipid and carbohydrate metabolism, changes in body structure, asthenia, obesity, decreased bone mineral density, impaired sexual function, mood swings . Before starting treatment, it is important to exclude other possible causes of the listed symptoms.
There are currently no clear age norms for testosterone values, but it should be noted that the physiological levels of serum testosterone begin to decline from about 30-40 years old, and the level of globulin increases. Determination of testosterone concentration should be carried out in the same laboratory.
Androgel is not used to treat erectile dysfunction or male infertility.
Before prescribing, the patient should be examined to exclude the presence of prostate cancer. In addition, regular monitoring of the breast and prostate gland should be carried out. In patients who are undergoing a long course of treatment, periodically it is necessary to check hematocrit, hemoglobin, lipid profile and functional liver tests.
There is evidence of an increased risk of developing sleep apnea in patients with hypogonadism who are being treated with testosterone esters.
In patients with severe liver failure, treatment with Androgel can lead to complications such as edema with or without congestive heart failure. Treatment in this case must be stopped immediately.
Some clinical symptoms, including nervousness, irritability, frequent or prolonged erections, weight gain, may indicate an excessive effect of androgen on the body, which requires dose adjustment.
It is recommended to maintain an interval of at least 6 hours between applying Androgel and taking a bath or shower.
Patients should observe the following precautions:
- Close the area of application of the gel after it has dried;
- Wash your hands with soap and water immediately after applying the preparation;
- Take a shower or bath before contact with a partner.
The drug is not used in women. In the case of pregnancy, the female partner should more carefully observe the safety measures, since the drug can have a virilizing effect on the fetus.
Androgel is not prescribed to patients who cannot comply with safety instructions (severe psychological disorders, chronic alcoholism or substance abuse).
Combined use of the drug with ACTH or corticosteroids may increase the risk of edema. Care must be taken when using these drugs together, especially in patients with kidney, liver and heart disease.
At the moment there is no data on how the use of Androgel can affect the patient's ability to drive vehicles and control complex mechanisms.
Analogs of Androgel are such drugs as Andriol and Nebido.
